Out of curiosity, how do most of you get media onto your server (e.g. ripping etc.)? What are your preferred applications for this and what are your preferred file formats for audio/video/other?

For DVD/Blu-rays I use the free version of DVDFab. For CDs I use Exact Audio Copy. For tagging I mainly use MP3Tag.
I rip everything asis, so a true 1:1 copy. For CDs I rip to FLAC and DVDs/Blu-rays I rip to ISO. Granted, the files take up a lot of space but disk space is cheap. The benefit of ripping everything asis is that you rip it once. If you need the music/video files in a different format further down the road you just convert the rips, you don't have to start again from scratch. Take it from someone who has been there done that, you only want to rip your media collection once.
